Before bringing your loved one home from the hospital complete this checklist.
Print this check list before taking your loved one home from the hospital. If they are being discharged to you to take home (firmly ask if they can leave on their own or are they being released because you are there, often they can't leave on their own, which puts you in charge) you should have this info.

How they will get there?
4. What is the diagnosis?
5. Have things progressed?
6. Why have the meds been changed? Is this a long term or short term med change? What signs should be watched for? If any of these signs happen do we call you ( Dr.) or 911?
7. Review with the Social Worker and/or the Dr. exactly what needs to be done BY the patient. Go over the BOUNDARIES that you need to set-up and make sure the patient understands and agrees to them before leaving.
Add any other questions that YOU need answered and get the answers before YOU agree to take that person home. If you have to be there to get them out/home, get these questions answered.
